United FC requests players who are interested in playing soccer at the college level complete this form.  The completed form will be directed to either Bryan Wallace (BU15-BU19) or Carrie Taylor (GU15-GU19).  The ADOC will then evaluate the player’s list of schools versus their athletic ability and make further recommendations.  The ADOC will then meet individually with the player to formulate a plan to accomplish their future goals.  Basic research is required to complete the form and we expect the player to take ownership of this process with the assistance of their parents.  Players should select a range of 10 potential colleges that they are interested in from Div. I to Junior College.
